CONCLUSIONES
Se presenta el manejo de
treinta pacientes sometidos a CO del maxilar superior y bimaxilar en forma
ambulatoria con ingreso hospitalario a las 6 AM y una estancia hospitalaria de
12 horas máximo.
Son múltiples los factores
a los que les atribuimos ésta posibilidad y son manejados en el preoperatorio
inmediato, transoperatorio y postoperatorio inmediato.
Estos factores son
corticoterapia, antibiotico terapia, técnica de Hipotensión Inducida, posición
de la cabeza durante cirugía, uso de anestésicos locales con vasoconstrictor,
método de irrigación, técnicas quirúrgicas estandarizadas, uso de
medicamentos antieméticos, eliminación de cerclaje intermaxilar, vendajes
compresivos, encaminados todos en conjunto a disminuir el sangrado y el tiempo
quirúrgico.
La principal ADF tratada
fue la Clase III Esquelética con Hipoplasia maxilar superior y el principal
procedimiento quirúrgico la Osteotomía Le Fort I.
No se presentaron
complicaciones transoperatorias o postoperatorias que contraindicaran el manejo
ambulatorio y ningún paciente requirió transfusión sanguínea.
Este manejo reduce los
costos para el paciente sin ser el objetivo principal del método.
